Why "Day 21" Is Wrong for Most People, and What One Number Can and Can't Tell You

Progesterone is made by the corpus luteum, the structure left behind in the ovary after an egg is released. Before ovulation there is almost none in the blood, under 1 ng/mL. After ovulation it climbs over several days, peaks in the middle of the luteal phase, and falls again just before the period starts unless a pregnancy has begun. That shape is the whole reason the test is useful, and the whole reason it is so often misread: a draw in the first half of the cycle, or on the day the period is about to start, returns a low number from a perfectly normal cycle.

The "day 21" instruction printed on many lab pages assumes a 28-day cycle with ovulation on day 14. If your cycle is 35 days, ovulation is around day 21 and progesterone will still be near zero on the day the instruction tells you to test. The rule that works for any cycle length is seven days before the next period is expected, or six to eight days after a positive ovulation predictor kit or a sustained temperature rise. Getting that right matters more than which lab you pick.
