Why this index exists. Public cash prices are the exception in American health care: PatientRightsAdvocate.org’s semi-annual compliance reports find that only about 21% of U.S. hospitals fully comply with the federal price-transparency rule. Direct-to-consumer labs are the rare corner of medicine that posts real prices on public pages — which is exactly what makes this index possible to build and verify. The approach owes a debt to ClearHealthCosts, whose pioneering crowdsourced cash-price journalism with newsroom partners demonstrated that publishing real prices changes what patients pay.
